Biomarker: F024-IgE Shrimp

Overview

F024-IgE Shrimp measures serum immunoglobulin E (IgE) antibodies directed against shrimp proteins, typically using a shrimp extract. IgE is an antibody involved in immediate-type allergic reactions, and shrimp tropomyosin is a major allergenic protein. A detectable result indicates sensitization to shrimp but does not by itself confirm clinical shrimp allergy or predict reaction severity. The test is clinically useful as part of an evaluation that includes the patient’s reaction history, physical examination, and, when needed, skin-prick testing or a medically supervised oral food challenge.

Clinical Use Cases

  • Supporting evaluation of suspected IgE-mediated shrimp or crustacean allergy.
  • Identifying shrimp sensitization when skin-prick testing cannot be performed because of extensive skin disease or interfering medications.
  • Complementing clinical history and skin-prick testing in the assessment of immediate reactions after shrimp ingestion.
  • Supporting evaluation of possible cross-sensitization involving other crustaceans, dust mites, or cockroaches, particularly when tropomyosin cross-reactivity is suspected.
  • Selecting patients who may require further assessment with component-resolved testing or a supervised oral food challenge.
  • Monitoring sensitization in research settings, although IgE concentration alone does not establish tolerance or predict reaction severity.

Specimen Types

  • Venous whole blood.
  • Serum separated from venous blood.
  • Plasma may be accepted by some assay systems, according to the testing laboratory’s validated requirements.

Measurement Methods

  • Automated in vitro allergen-specific IgE immunoassay.
  • Fluoroenzyme immunoassay or comparable enzyme-linked immunoassay using shrimp extract.
  • Component-resolved immunoassay for shrimp allergens such as tropomyosin, including Pen a 1 or Pen m 1, when available.
  • Immunoblotting or other research immunoassays for identifying IgE binding to individual shrimp proteins.
  • Skin-prick testing is a related clinical allergy test but does not measure circulating serum IgE.

Test Preparation and Influencing Factors

  • Fasting is generally not required for a serum shrimp-specific IgE test.
  • Antihistamines usually do not alter serum-specific IgE measurements, although they can suppress skin-prick test responses.
  • Results should be interpreted with the timing, reproducibility, and characteristics of symptoms after shrimp exposure.
  • A positive result may represent sensitization without clinical allergy; cross-reactivity with dust mites, cockroaches, or other invertebrates can contribute to positive results.
  • The measured IgE concentration does not reliably predict the severity of a future reaction.
  • Assay results may vary with the shrimp species, extract composition, allergen preparation, and laboratory method.
  • A negative result does not absolutely exclude shrimp allergy when the clinical history is strongly suggestive.
  • The suspected food should not be deliberately reintroduced based only on the test result. When diagnosis remains uncertain, an oral food challenge should be conducted only under appropriate medical supervision.
