Biomarker: F089-IgE Mustard

Overview

F089-IgE Mustard measures immunoglobulin E (IgE) antibodies in blood that react to mustard allergens. IgE is an antibody involved in immediate-type allergic reactions, including the release of mediators that can cause itching, hives, gastrointestinal symptoms, respiratory symptoms, or anaphylaxis. A detectable or elevated mustard-specific IgE result indicates sensitization, but it does not by itself confirm clinical allergy or predict reaction severity. The test is clinically useful when interpreted with the exposure history, symptoms, and, when necessary, supervised skin testing or oral food challenge.

Clinical Use Cases

  • Supports evaluation of suspected IgE-mediated mustard allergy.
  • Helps investigate immediate allergic symptoms after consuming mustard or foods containing mustard.
  • Assesses mustard sensitization in patients with suspected food allergy or unexplained anaphylaxis.
  • Contributes to evaluation of possible cross-sensitization involving related plant foods or mugwort pollen.
  • Supports research on mustard allergens, including Sin a 1 and Sin a 2, and their relationship to clinical reactions.
  • Helps guide further diagnostic assessment, but should not be used alone to establish a mustard-allergy diagnosis.

Specimen Types

  • Serum, typically collected from venous blood.
  • Plasma may be accepted by some assay systems, depending on the laboratory method.

Measurement Methods

  • In-vitro allergen-specific IgE immunoassay using mustard extract.
  • Fluorescence enzyme immunoassay or equivalent solid-phase immunoassay.
  • Radioallergosorbent test (RAST), used in earlier clinical studies.
  • Enzyme-linked immunosorbent assay (ELISA), primarily used in research and for characterization of allergen-specific responses.
  • Immunoblotting, primarily used in research to identify IgE-binding mustard proteins such as Sin a 1 and Sin a 2.
  • Skin-prick testing and supervised oral food challenge may complement blood measurement during clinical evaluation, but they are separate tests.

Test Preparation and Influencing Factors

  • Fasting is generally not required for an allergy blood test.
  • Patients should report all medications and supplements. Some medications may affect associated allergy testing, and medication changes should occur only when directed by a healthcare professional.
  • The result should be interpreted with the timing, amount, and form of mustard exposure and the patient’s symptoms.
  • A positive result indicates sensitization and does not necessarily mean that ingestion will cause symptoms.
  • The concentration of mustard-specific IgE does not reliably predict reaction severity.
  • Total IgE concentration, age, atopic conditions, and assay-specific reference ranges may affect interpretation.
  • Cross-reactivity with related plant allergens, including mugwort pollen and other Brassicaceae foods, may contribute to sensitization patterns.
  • Extract composition and the mustard species tested may influence results because yellow and brown mustard contain different allergenic proteins.
  • A negative result reduces, but does not completely eliminate, the possibility of clinically relevant IgE-mediated allergy.
  • Oral food challenges should be performed only under appropriate medical supervision because they can provoke allergic reactions.
