Biomarker: F233-IgE Ovomucoid

Overview

F233-IgE Ovomucoid measures serum immunoglobulin E (IgE) antibodies directed against ovomucoid, also known as Gal d 1, a major glycoprotein allergen in hen’s egg white. Ovomucoid has trypsin-inhibitor activity and is relatively resistant to heat and digestion, which may contribute to reactions to cooked or baked egg. Elevated ovomucoid-specific IgE indicates sensitization but does not independently confirm clinical egg allergy or predict reaction severity. The test is clinically useful as part of an allergist’s evaluation, particularly when assessing persistent egg allergy or the likelihood of reacting to heated egg, although egg-white-specific IgE and supervised oral food challenge may provide additional or more definitive information.

Test Preparation and Influencing Factors

  • Fasting is generally not required for an allergy blood test.
  • Patients should report all prescription medicines, over-the-counter medicines, and supplements. Medications should not be stopped unless instructed by a health care professional.
  • Antihistamines generally do not alter serum-specific IgE measurement, although they may affect skin-prick testing. The testing clinician should provide medication-specific instructions.
  • Results should be interpreted with the patient’s reaction history, age, diet, other egg-specific IgE results, and clinical examination.
  • A positive result indicates sensitization and does not by itself establish symptomatic allergy or predict reaction severity.
  • Ovomucoid-specific IgE concentrations and clinical decision points vary by population, assay, age, and egg preparation. Published thresholds should not be transferred between assays without clinical validation.
  • A negative or low result does not completely exclude egg allergy, especially when the clinical history is strongly suggestive.
  • Oral food challenges, when needed, must be performed under medical supervision because allergic reactions can be severe.
