Biomarker: M008-IgE Setomelanomma rostrat
Overview
M008-IgE measures serum immunoglobulin E (IgE) antibodies directed against the mold historically identified as Setomelanomma rostrata, also associated with Helminthosporium halodes and Exserohilum rostratum. Specific IgE indicates immune sensitization to allergen-containing mold extracts, which may be relevant to allergic rhinitis, asthma, or other mold-associated respiratory symptoms. A high result supports sensitization but does not by itself establish that exposure causes symptoms; a low or undetectable result does not completely exclude mold allergy. Interpretation requires correlation with clinical history, exposure timing, examination, and other allergy testing because mold extracts are not uniformly standardized and recognized species-specific molecular allergens are limited.
Clinical Use Cases
- Supports evaluation of suspected IgE-mediated sensitization to Setomelanomma or related mold extracts in patients with compatible rhinitis, asthma, sinus, or other respiratory symptoms.
- Helps identify mold sensitization as part of a targeted inhalant-allergen evaluation.
- May contribute to assessment of patients with suspected mold-associated allergic respiratory disease when interpreted with clinical exposure history.
- May be included in broader mold-allergen panels or research studies evaluating inhalant allergen sensitization.
- Does not diagnose environmental mold exposure, toxic mold illness, or clinical allergy on its own.
Specimen Types
- Serum obtained from venous blood.
- Skin-prick or puncture testing may be used as an alternative form of assessment when a suitable extract is available.
Measurement Methods
- In-vitro serum allergen-specific IgE immunoassay.
- Fluorescence enzyme immunoassay or a comparable solid-phase immunoassay.
- Qualitative or semiquantitative reporting in allergen-specific IgE units, commonly ( \mathrm{kU/L} ) or ( \mathrm{kUA/L} ).
- Skin-prick or puncture testing with mold extract as an in-vivo alternative.
- Results from different assay platforms and extracts should not be assumed to be directly interchangeable because mold-allergen extracts may have limited standardization.
Test Preparation and Influencing Factors
- No fasting is generally required for a serum specific-IgE blood test.
- Antihistamines generally do not interfere with serum specific-IgE testing, but medications should still be reported to the healthcare provider.
- Antihistamines can suppress skin-test reactions and may need to be withheld before skin testing according to the clinician’s instructions.
- Collection timing usually does not require a specific relationship to mold exposure.
- A positive result indicates sensitization, not necessarily symptomatic allergy; interpretation should include symptoms, exposure history, age, total IgE, and other test findings.
- Mold extracts may differ in composition, potency, and standardization, which can affect sensitivity and specificity.
- Cross-reactivity with other fungal allergens may contribute to a positive result, and species-specific molecular allergens for this mold are not currently well established.
