Biomarker: Immunoglobulin E, Total

Overview

Total immunoglobulin E (IgE) measures the overall concentration of IgE antibodies circulating in the blood, regardless of their specificity for particular allergens. IgE is an antibody involved in type I hypersensitivity reactions and plays a central role in allergic responses such as asthma, allergic rhinitis, and atopic dermatitis. Elevated total IgE levels are commonly associated with atopic allergic diseases, parasitic infections, certain primary immunodeficiencies, and allergic bronchopulmonary aspergillosis, while normal or low levels do not exclude IgE‑mediated disease. Clinically, total IgE is used as an adjunct test to support diagnosis or monitoring in selected conditions and to help guide anti‑IgE therapies, but it is not a stand‑alone screening test for allergy.

Clinical Use Cases

  • Assessing overall IgE burden in patients with suspected or known atopic diseases such as allergic asthma, allergic rhinitis, and atopic dermatitis.
  • Supporting diagnosis and follow‑up of allergic bronchopulmonary aspergillosis (ABPA) in appropriate clinical contexts.
  • Evaluating for parasitic (especially helminth) infections when clinically suspected, in conjunction with other diagnostic tests.
  • Assessing certain primary immunodeficiency disorders and hyper‑IgE syndromes, where markedly elevated IgE can be a characteristic feature.
  • Contributing to risk stratification or follow‑up in other conditions with IgE dysregulation, including some inflammatory diseases and malignancies, as part of a broader work‑up.
  • Determining eligibility and dosing ranges for anti‑IgE biologic therapies (for example, omalizumab) in asthma and other IgE‑mediated conditions.

  • Plasma from venous blood (in some laboratory protocols).

Measurement Methods

  • Immunoassay techniques such as enzyme‑linked immunosorbent assay (ELISA).
  • Automated fluorescent or chemiluminescent immunoassays used in clinical laboratories.
  • Other quantitative immunoassays validated for total serum IgE measurement.

Test Preparation and Influencing Factors

  • Typically no fasting requirement; total IgE is usually measured on a routine blood sample.
  • Recent or ongoing parasitic infections, atopic diseases, and inflammatory conditions can increase total IgE and influence interpretation.
  • Primary immunodeficiency syndromes and hyper‑IgE states can markedly elevate levels and should be considered when IgE is very high.
  • Age‑related reference ranges vary, with higher normal ranges often seen in children than adults, so age must be considered when interpreting results.
  • Concomitant use of biologic anti‑IgE therapies (such as omalizumab) and other immunomodulatory treatments can alter circulating IgE measurements and complicate interpretation.
  • Total IgE can be elevated by diverse conditions and normal values do not rule out IgE‑mediated allergy, so clinical context and specific IgE tests or skin tests are important.
