Biomarker: W009-IgE Plantain, English

Overview

This biomarker measures serum allergen-specific immunoglobulin E (IgE) antibodies directed against English plantain (Plantago lanceolata) pollen. IgE is involved in immediate hypersensitivity reactions by binding allergens and activating mast cells and basophils, which can produce symptoms such as allergic rhinitis, conjunctivitis, and asthma. A detectable or elevated result indicates sensitization to plantain pollen, while a low or undetectable result indicates that plantain-specific IgE was not detected by the assay. Sensitization alone does not establish clinically relevant allergy, and the result does not predict symptom severity. Testing is clinically useful for evaluating suspected seasonal weed-pollen allergy and, in selected cases, distinguishing genuine plantain sensitization from IgE cross-reactivity with other pollens.

Measurement Methods

  • Allergen-specific immunoassay using Plantago lanceolata pollen extract immobilized on a solid phase, such as a fluorescence enzyme immunoassay.
  • Enzyme-linked immunosorbent assay, or ELISA, in research settings.
  • Immunoblotting for characterization of IgE-binding plantain pollen proteins in research settings.
  • Multiplex allergen microarray testing for component-resolved IgE measurement, including Pla l 1 where available.
  • Skin-prick testing with plantain pollen extract is a complementary in-vivo method, but it does not measure serum IgE directly.

Test Preparation and Influencing Factors

  • Fasting is generally not required for an allergy-specific IgE blood test.
  • Patients should report all medications, particularly antihistamines and other allergy treatments. Medication changes should be made only under the direction of a healthcare professional.
  • The test can be collected without regard to the current pollen season, although interpretation should consider the patient’s exposure history and symptom timing.
  • Results should be interpreted with symptoms, exposure history, physical findings, and other allergy tests because serum-specific IgE demonstrates sensitization and does not by itself prove clinical allergy.
  • Cross-reactivity with related pollen allergens, including grass, birch, ash, and other weed pollens, may produce positive extract-based results that do not represent genuine plantain sensitization.
  • Pla l 1 is considered a marker of genuine English plantain sensitization, but its clinical value may vary by population and diagnostic context.
  • Total IgE concentration, smoking, parasitic infection, and other atopic conditions can influence broader IgE interpretation but do not specifically establish plantain allergy.
  • Serum should be handled according to the validated assay protocol. For one CDC-documented specific-IgE method, serum from red-top or serum-separator tubes was used, repeated freeze-thaw cycles were avoided, and samples were stored frozen for longer-term preservation.
