Biomarker: W016-IgE Rough Marshelder
Overview
W016-IgE Rough Marshelder measures serum immunoglobulin E (IgE) antibodies specific to rough marshelder pollen, an inhaled weed pollen associated with the Iva genus. IgE is involved in immediate hypersensitivity reactions, including allergic rhinitis, allergic conjunctivitis, and asthma. A high result indicates sensitization to the tested allergen, while a low or undetectable result indicates that significant circulating specific IgE was not detected by the assay. The result supports evaluation of suspected seasonal respiratory allergy but does not, by itself, establish symptomatic allergy or predict reaction severity.
Clinical Use Cases
- Evaluate suspected seasonal allergic rhinitis, allergic conjunctivitis, or pollen-associated asthma.
- Identify sensitization to rough marshelder pollen when symptoms occur during relevant pollen exposure.
- Support allergen avoidance and environmental exposure counseling.
- Complement clinical history and, when appropriate, skin-prick testing.
- Provide an alternative to skin testing when skin testing is unsuitable or antihistamines cannot be discontinued.
Specimen Types
- Serum.
- Plasma may be acceptable for some validated immunoassays.
- Venous whole blood is collected before serum or plasma is separated.
Measurement Methods
- Allergen-specific immunoassay using a rough marshelder pollen extract.
- Fluorescent enzyme immunoassay.
- Enzyme-linked immunosorbent assay.
- Chemiluminescent immunoassay.
- Radioallergosorbent testing is an older method and has largely been replaced by newer immunoassays.
Test Preparation and Influencing Factors
- Fasting is generally not required for a serum allergy blood test.
- Routine serum specific-IgE testing is generally not affected by antihistamines, unlike skin-prick testing. Patients should follow the ordering clinician’s medication instructions and should not stop prescribed medicines without medical advice.
- Recent or ongoing exposure to pollen may be relevant when interpreting results, but testing is usually guided by the patient’s symptom history rather than by a required collection date.
- Total IgE concentration, age, atopic dermatitis, and other sensitizations may affect interpretation.
- A positive result indicates sensitization, not necessarily clinical allergy. Interpretation should consider symptoms, timing of exposure, physical findings, and other allergy-test results.
- The concentration of specific IgE does not reliably predict the severity of a future allergic reaction.
- Assay methods, reporting units, reference ranges, and decision thresholds may differ between laboratories.
