Biomarker: E006-IgE Guinea Pig Epithelium

Overview

E006-IgE Guinea Pig Epithelium measures allergen-specific immunoglobulin E (IgE) antibodies in blood directed against guinea pig epithelial or dander allergens. IgE participates in immediate hypersensitivity reactions by activating mast cells and basophils, which can cause rhinitis, conjunctivitis, wheezing, asthma, urticaria, or, rarely, anaphylaxis. A detectable or elevated result indicates sensitization and may support guinea pig allergy when consistent with exposure history and symptoms, but it does not by itself confirm clinical allergy or predict reaction severity. The test is clinically useful for identifying guinea pig as a possible allergen and for distinguishing sensitization to guinea pig from cross-reactivity with cat or dog allergens.

Clinical Use Cases

  • Evaluation of suspected allergy after exposure to guinea pigs, their dander, hair, urine, or cage environments.
  • Investigation of allergic rhinitis, conjunctivitis, asthma, wheezing, urticaria, or other immediate symptoms associated with guinea pig exposure.
  • Identification of the allergen responsible for symptoms when several animal exposures are possible.
  • Assessment of occupational allergy in laboratory animal workers or other individuals with repeated animal exposure.
  • Supporting allergy evaluation when skin testing is unsuitable because of skin disease, medication use, or concern about provoking a reaction.
  • Assessment of sensitization before allergen immunotherapy when guinea pig allergen is clinically relevant.
  • Research on guinea pig allergy, allergen sensitization, and IgE cross-reactivity among furry animals.

Specimen Types

  • Serum from venous blood.
  • Plasma may be accepted by some assay systems, but specimen requirements are method- and laboratory-dependent.

Measurement Methods

  • Fluorescence enzyme immunoassay, including ImmunoCAP-specific IgE testing.
  • Enzyme-linked immunosorbent assay, used in clinical research and some laboratory-developed methods.
  • Immunoassays using whole guinea pig epithelial or dander extracts.
  • Component-resolved immunoassays measuring IgE to individual guinea pig allergens, such as Cav p 1, Cav p 2, Cav p 3, Cav p 4, or Cav p 6.

Test Preparation and Influencing Factors

  • Fasting is generally not required for an allergen-specific IgE blood test.
  • Patients should report all medications and supplements. Antihistamines usually have less effect on blood testing than on skin testing, but medication instructions should come from the ordering clinician.
  • Results should be interpreted with the patient’s exposure history, symptoms, physical findings, and other allergy testing.
  • A positive result indicates sensitization and does not necessarily indicate symptomatic allergy.
  • The concentration of specific IgE does not reliably predict the severity of a future allergic reaction.
  • Cross-reactivity with cat, dog, or other mammalian allergens can produce positive results, particularly through shared lipocalins or serum albumins.
  • Markedly elevated total IgE may contribute to nonspecific binding and false-positive results.
  • Recent exposure is not required for testing, but the timing and pattern of symptoms relative to guinea pig exposure are important for interpretation.
  • Immunotherapy may alter specific IgE results; specific IgE testing is not generally useful for determining whether clinically relevant sensitivity remains after immunotherapy.
