Biomarker: Chloride, Urine

Overview

Urine chloride measures the amount of chloride excreted in urine, usually in a spot sample or a 24-hour collection. Chloride is a major body electrolyte involved in fluid balance and acid-base regulation, so urine chloride helps clinicians assess kidney handling of salt and the body’s response to volume or acid-base disturbances. High urine chloride may be seen with salt-wasting states, adrenal insufficiency, polyuria, or high salt intake, while low urine chloride may occur with vomiting, diarrhea, Cushing syndrome, SIADH, or low salt intake. The test is clinically useful for evaluating metabolic alkalosis, volume status, and some renal, adrenal, and electrolyte disorders.

  • Evaluation of metabolic alkalosis, especially to distinguish chloride-responsive from chloride-unresponsive causes.
  • Assessment of volume depletion and renal handling of sodium chloride in fluid and electrolyte disorders.
