Biomarker: Chloride, blood
Overview
Blood chloride measures the concentration of chloride ions in serum or plasma, the major negatively charged ion in extracellular fluid.
Chloride helps maintain fluid balance, osmotic pressure, and acid base (pH) balance, and works closely with sodium and bicarbonate in these roles.
High levels (hyperchloremia) are associated with conditions such as dehydration, metabolic acidosis, kidney dysfunction, and excessive saline administration.
Low levels (hypochloremia) can occur with overhydration, chronic respiratory disease, heart failure, prolonged vomiting, diarrhea, or certain kidney and hormonal disorders.
Clinically, serum chloride is a routine electrolyte measured within metabolic panels to help assess hydration status, kidney function, and acid base disorders.
Clinical Use Cases
- Evaluation of water and electrolyte balance, including assessment of dehydration or overhydration.
- Assessment and monitoring of acid base disorders such as metabolic acidosis and metabolic alkalosis.
- Part of routine testing to evaluate kidney function and kidney-related disorders, including acute kidney injury and chronic kidney disease.
- Monitoring in heart failure and other conditions affecting fluid status and circulation.
- Evaluation of liver disease and related fluid and electrolyte disturbances.
- Investigation of symptoms such as vomiting, diarrhea, fatigue, weakness, dehydration, and breathing difficulties that may reflect fluid or acid base imbalance.
- Monitoring electrolyte effects of medications such as diuretics, corticosteroids, bicarbonates, laxatives, and carbonic anhydrase inhibitors.
- Use in critical care and emergency settings, often with point of care testing, to guide rapid management of electrolyte and acid base imbalances.
- Contribution to diagnosis and management of metabolic disorders such as diabetic ketoacidosis and renal tubular acidosis.

- Automated serum or plasma electrolyte analyzers using ion selective electrodes for chloride.

Test Preparation and Influencing Factors
- Usually no special preparation or fasting is required for a chloride blood test; it is commonly done as part of routine panels.
- Hydration status (dehydration or overhydration) can significantly affect chloride levels.
- Acute gastrointestinal losses such as vomiting, diarrhea, or gastric suction can lower chloride and alter acid base balance.
- Kidney function abnormalities (acute kidney injury, chronic kidney disease, renal tubular acidosis, kidney failure) influence chloride retention or loss.
- Medications including diuretics, corticosteroids, bicarbonates/antacids, laxatives, carbonic anhydrase inhibitors, and saline infusions can raise or lower chloride levels.
- Hormonal and endocrine conditions such as Addison disease, aldosteronism, and Cushing syndrome can affect chloride through changes in sodium and water handling.
- Respiratory disorders and chronic respiratory acidosis or alkalosis can change chloride as part of compensatory mechanisms.
- Dietary salt intake and consumption of saline or salt water may increase chloride levels.
