Biomarker: Calcium, Urine
Overview
A urine calcium test measures the amount of calcium excreted in urine, usually in a 24 hour collection, and sometimes in a random (spot) sample.
Calcium is essential for bone structure, neuromuscular function, and many cellular processes, and urinary calcium reflects how the body handles calcium through the gut, bones, kidneys, and parathyroid hormone regulation.
High urine calcium (hypercalciuria) is commonly associated with calcium kidney stone risk, primary hyperparathyroidism, some bone diseases, and conditions affecting kidney handling of calcium.
Low urine calcium can be seen with hypoparathyroidism, low dietary calcium intake, malabsorption, or certain kidney and endocrine disorders.
Clinically, urine calcium is useful for evaluating causes of abnormal blood calcium, assessing kidney stone risk, and helping differentiate disorders of calcium and parathyroid function.
Clinical Use Cases
- Evaluate risk of calcium-containing kidney stones and guide prevention strategies.
- Assess and monitor primary hyperparathyroidism and other parathyroid disorders, including distinguishing primary hyperparathyroidism from familial hypocalciuric hypercalcemia when combined with blood tests.
- Help investigate causes of hypercalcemia or hypocalcemia when blood calcium is abnormal, by assessing renal calcium excretion.
- Evaluate and monitor bone diseases such as osteoporosis and osteomalacia, including response to calcium and vitamin D supplementation.
- Assess kidney function and tubular calcium handling in various renal disorders.
- Assist in the evaluation of unexplained hematuria, abdominal or flank pain, or pediatric stone disease, where hypercalciuria may be a contributing factor.
- Monitor patients on therapies that affect calcium balance, such as vitamin D, calcium supplements, or certain diuretics, for development or control of hypercalciuria.

Measurement Methods
- Quantitative colorimetric assays for calcium on automated chemistry analyzers.
- Ion-selective electrode methods for calcium on clinical chemistry platforms (less common for urine than for serum but described in laboratory practice).
- Calculation of calcium to creatinine ratio in spot urine as an estimate of calcium excretion, especially in children or when 24 hour collection is not feasible.
Test Preparation and Influencing Factors
- Collection duration: For a 24 hour test, all urine over a full 24 hour period must be collected; missed samples can invalidate results.
- Dietary calcium intake: High or low calcium intake, including supplements and dairy, significantly influences urinary calcium and should be considered when interpreting results.
- Dietary sodium and protein: High sodium and high animal protein intake increase urinary calcium excretion and may elevate results.
- Medications: Thiazide diuretics lower urine calcium, while loop diuretics, some antacids, vitamin D, and high-dose calcium supplements can increase it.
- Hydration status: Very high or low fluid intake changes urine volume and calcium concentration, affecting interpretation.
- Timing of collection: For spot tests, laboratories may prefer fasting or specific postprandial timing; fasting or post-load samples may be used to characterize types of hypercalciuria in some protocols.
- Pregnancy, growth, and age: Physiologic changes in calcium metabolism and age-specific reference ranges, especially in children, affect result interpretation.
- Kidney function: Chronic kidney disease and other renal disorders alter calcium handling and can affect urinary calcium levels.
