Overview

Total cholesterol is a blood biomarker that measures the sum of all cholesterol carried in your blood by different lipoprotein particles, mainly LDL, HDL, and VLDL. Cholesterol is a waxy fat‑like substance used to build cell membranes and some hormones, but too much can contribute to plaque buildup in arteries and raise the risk of heart attack and stroke. In practical terms, higher total cholesterol usually suggests greater cardiovascular risk, while lower values are generally associated with lower risk, although clinicians always interpret this number along with LDL, HDL, and triglycerides. This test is useful because it is simple, widely available, and helps screen for heart‑related risk and guide lifestyle or medication decisions.

  • Screening for cardiovascular disease risk in adults with or without symptoms.
  • Monitoring people with known heart disease, diabetes, or high blood pressure.
  • Assessing metabolic risk and guiding diet, exercise, or lipid‑lowering therapy (such as statins).
  • Evaluating overall lipid patterns within a full lipid panel (LDL, HDL, triglycerides).
  • Tracking how well cholesterol‑lowering treatments are working over time.

  • High‑performance liquid chromatography with tandem mass spectrometry (LC‑MS/MS) for very precise, specialized measurements.
  • Gas chromatography–mass spectrometry (GC‑MS) in research or reference‑laboratory settings.

Test Preparation and Influencing Factors

  • Fasting for about 9–12 hours is often recommended before a lipid panel, although evidence suggests that changes in total cholesterol between fasting and non‑fasting values are usually small for most people.
  • Recent large meals or heavy alcohol intake can raise triglycerides and modestly affect total cholesterol interpretation.
  • Certain medications (such as statins, fibrates, some steroids, and some blood‑pressure drugs) can lower or raise cholesterol levels.
  • Pregnancy, acute illness (for example, major infection or recent heart attack), and rapid weight loss can transiently lower cholesterol.
  • Timing of the menstrual cycle, stress, and severe thyroid or liver disease may also influence results.
