Biomarker: Calcium, Ionized, Serum

Overview

Serum ionized calcium measures the concentration of free (unbound) calcium ions in the blood that are not attached to proteins such as albumin. This free fraction is the physiologically active form of calcium and is essential for normal nerve conduction, muscle contraction, cardiac function, and blood clotting. High ionized calcium levels are associated with conditions such as hyperparathyroidism, hyperthyroidism, certain malignancies, granulomatous diseases, and vitamin D or A excess, while low levels can indicate hypoparathyroidism, vitamin D deficiency, kidney failure, pancreatitis, malabsorption, or bone mineralization disorders. Ionized calcium testing is clinically useful when total calcium is abnormal or unreliable, in critically ill patients, and in settings where rapid and accurate assessment of calcium status is needed, such as surgery, intensive care, and kidney disease management.

Clinical Use Cases

  • Evaluation of suspected hypercalcemia or hypocalcemia, especially when total calcium is borderline or discordant with symptoms.
  • Assessment and monitoring of parathyroid disorders (hyperparathyroidism, hypoparathyroidism).
  • Monitoring calcium balance in chronic kidney disease and other conditions affecting calcium and phosphate metabolism.
  • Management of critically ill patients, including those in intensive care units, where protein binding and acid–base disturbances alter total calcium.
  • Perioperative monitoring during major surgery, such as liver transplantation and cardiopulmonary bypass, where rapid shifts in ionized calcium occur.
  • Monitoring patients receiving massive or rapid transfusions of citrated blood products, which can lower ionized calcium and impair cardiac function.
  • Evaluation of calcium status in neonates and children, including neonatal hypocalcemia.
  • Investigation of calcium-related complications in malignancy, including multiple myeloma and metastatic bone disease.
  • Assessment of calcium balance in vitamin D–related disorders and granulomatous diseases such as sarcoidosis.

Specimen Types

  • Serum (most common for this biomarker specification).
  • Heparinized whole blood (often used in point-of-care or critical care settings for rapid ionized calcium measurement).
  • Plasma (lithium heparin), in some laboratory protocols.

Measurement Methods

  • Ion-selective electrode (ISE) analysis, typically performed on blood gas or electrolyte analyzers and considered the standard method for ionized calcium.
  • Direct potentiometry using calcium-selective membranes within automated analyzers.
  • pH-corrected reporting of ionized calcium (standardized to pH 7.40) to account for pH-dependent binding to proteins.

Test Preparation and Influencing Factors

  • Fasting is not always required, but some laboratories may request a fasting sample to minimize postprandial changes. MedlinePlus notes that preparation instructions depend on the specific lab protocol.
  • Medications such as thiazide diuretics, vitamin D or A supplements, lithium, and certain antacids or calcium supplements can alter ionized calcium levels and should be discussed with the clinician before testing.
  • Acid–base status significantly affects ionized calcium; acidosis increases ionized calcium, while alkalosis decreases it, because protein binding changes with pH.
  • Serum albumin and total protein affect total calcium but not ionized calcium directly; ionized measurements are preferred when protein levels are abnormal (for example, in critical illness, liver disease, or nephrotic syndrome).
  • Timing and handling of the specimen are critical; delayed analysis, exposure of the sample to air, or improper sealing can alter pH and thus ionized calcium results.
  • Massive transfusion of citrated blood products can lower ionized calcium by chelation, requiring close monitoring.
  • Physiological states such as pregnancy, severe illness, and rapid changes in ventilation (altering CO₂ and pH) can influence measured ionized calcium.
