Biomarker: Carbon Dioxide
Overview
Carbon dioxide (CO₂) testing in clinical practice typically measures either the total CO₂ (mainly bicarbonate, HCO₃⁻) in serum or plasma or the partial pressure of CO₂ (PaCO₂) in arterial blood. Total CO₂ reflects the bicarbonate level, which is a major component of the body’s acid base balance and is routinely included in basic and comprehensive metabolic panels. Arterial PaCO₂ directly indicates the adequacy of ventilation and is a key parameter in arterial blood gas (ABG) analysis used to assess respiratory function and acid base status. Abnormally high or low CO₂ values can indicate respiratory disorders, kidney disease, metabolic acidosis or alkalosis, and other conditions that disturb acid base homeostasis. Clinically, CO₂ measurements help diagnose and monitor lung disease, kidney disease, electrolyte and acid base disturbances, and guide acute and chronic treatment decisions.

- Assessment of acid base balance as part of a basic or comprehensive metabolic panel (serum total CO₂ / bicarbonate).
- Evaluation of electrolyte and metabolic disturbances, such as metabolic acidosis or metabolic alkalosis.
- Monitoring and diagnosis of kidney disease, including chronic kidney disease and renal tubular disorders that affect bicarbonate handling.
- Evaluation and management of lung and respiratory disorders (for example COPD, asthma exacerbations, respiratory failure) using PaCO₂ on arterial blood gases.
- Monitoring of ventilation status in critically ill or mechanically ventilated patients via PaCO₂.
- Assessment of response to treatment for conditions affecting breathing, acid base balance, or kidney function (for example oxygen therapy, ventilator adjustments, bicarbonate therapy).
- Preoperative or emergency evaluation of patients with suspected respiratory or metabolic derangements using ABG analysis.

- Arterial blood for arterial blood gas analysis measuring PaCO₂.

- Arterial blood gas analyzers measuring pH, PaCO₂, and PaO₂ using potentiometric and related electrode-based methods.
Test Preparation and Influencing Factors
- CO₂ (bicarbonate) testing is commonly performed on nonfasting samples, although fasting may be required if measured within a broader metabolic panel that includes glucose.
- Medications such as diuretics, corticosteroids, laxatives, and bicarbonate-containing drugs can alter CO₂ levels by affecting fluid, electrolyte, or acid base balance.
- Severe vomiting or diarrhea can significantly change bicarbonate and CO₂ levels by causing loss of gastric or intestinal fluids.
- Respiratory rate and depth (hyperventilation or hypoventilation) directly influence PaCO₂ and thus ABG results.
- Chronic lung diseases (for example COPD) and conditions causing hypoventilation can lead to chronically elevated PaCO₂ and secondary metabolic compensation.
- High altitude can lower normal PaCO₂ due to ventilatory stimulation, so local reference values may differ.
- Improper sample handling (delays in analysis, exposure of blood to air) may alter measured CO₂ and PaCO₂ values.
