Biomarker: Glucose, urine

Overview

Urine glucose measures the amount of glucose excreted in urine, a finding known as glycosuria or glucosuria.
Glucose appears in urine when filtered glucose exceeds the kidney’s reabsorptive capacity or when renal tubular reabsorption is impaired.
High urine glucose levels are most commonly associated with diabetes mellitus, but can also occur in pregnancy and certain kidney disorders such as renal glycosuria or Fanconi syndrome.
Urine glucose testing is clinically useful as part of urinalysis for general health screening, to flag possible diabetes or renal tubular disorders, and in situations where blood testing is difficult.

Clinical Use Cases

  • Screening for possible diabetes mellitus or gestational diabetes when blood testing is not available or feasible.
  • Part of routine urinalysis to assess general health, especially when evaluating symptoms suggestive of diabetes or kidney disease.
  • Initial evaluation of glycosuria to differentiate between hyperglycemia-related causes and primary renal tubular dysfunction (renal glycosuria, Fanconi syndrome).
  • Monitoring of diabetes or risk of diabetes in selected patients using at-home urine glucose test strips when blood glucose testing is difficult.
  • Assessment during pregnancy as part of prenatal care urinalysis to identify possible gestational diabetes.

Specimen Types

  • Spot (random) urine sample.
  • Clean-catch midstream urine sample as part of standard urinalysis.
  • Occasionally timed or 24‑hour urine collection in specialized evaluations.

Measurement Methods

  • Urine dipstick test using glucose oxidase–based reagent strips (colorimetric test).
  • Semi‑quantitative or qualitative automated urinalysis analyzers using reagent strip chemistry.
  • Older chemical reduction methods (such as Benedict’s test), now largely replaced by glucose oxidase methods.

Test Preparation and Influencing Factors

  • Typically no special preparation; urine glucose testing is often done on a routine sample.
  • Recent high‑carbohydrate meals can cause mild, transient increases in urine glucose in people with diabetes or impaired glucose tolerance.
  • Pregnancy can increase urine glucose due to physiological changes in renal glucose handling and may indicate gestational diabetes in some cases.
  • Serum glucose level above the renal threshold (around 180 mg/dL in many adults) increases the likelihood of glucose appearing in urine.
  • Renal tubular disorders (such as renal glycosuria or Fanconi syndrome) cause urine glucose despite normal blood glucose.
  • Certain medications, including sodium–glucose cotransporter 2 (SGLT2) inhibitors, increase urinary glucose excretion even with normal blood glucose.
  • Ascorbic acid (vitamin C) may interfere with some dipstick methods and contribute to false‑negative results.
