Overview

Glucose is a simple sugar that serves as the primary energy source for human cells and is transported in the bloodstream to tissues throughout the body.
Blood glucose levels reflect the balance between dietary intake, hepatic glucose production, and uptake by tissues under hormonal regulation, mainly insulin and glucagon.
High blood glucose (hyperglycemia) is a key feature of diabetes and prediabetes, while low blood glucose (hypoglycemia) can cause symptoms such as shakiness, confusion, and, in severe cases, loss of consciousness.
Clinically, measuring glucose is central to screening, diagnosing, and monitoring diabetes, prediabetes, and gestational diabetes and to assessing acute metabolic disturbances.

Clinical Use Cases

  • Screening for type 2 diabetes and prediabetes using fasting plasma glucose, oral glucose tolerance tests, and A1C-related measures.
  • Diagnosis and monitoring of type 1 diabetes and type 2 diabetes through fasting blood glucose, random blood glucose, and home blood glucose monitoring.
  • Screening and diagnosis of gestational diabetes in pregnancy with oral glucose tolerance tests and glucose challenge tests.
  • Monitoring day‑to‑day glycemic control in people with diabetes using capillary blood glucose meters and continuous glucose monitoring (where available).
  • Assessment of acute hyperglycemia or hypoglycemia in emergency and inpatient settings.
  • Evaluation of treatment effectiveness and risk of diabetes complications using blood glucose tests in combination with hemoglobin A1C.

Specimen Types

  • Venous blood (plasma or serum) for fasting blood glucose, random glucose, oral glucose tolerance tests, and A1C-related measures.
  • Capillary whole blood (fingerstick) for point‑of‑care and home blood glucose monitoring.
  • Urine for glucose in urine tests, used less commonly because of lower accuracy compared with blood tests.

Measurement Methods

  • Automated laboratory measurement of plasma or serum glucose using enzymatic methods (commonly glucose oxidase or hexokinase) on chemistry analyzers.
  • Fasting blood glucose (FBG) / fasting plasma glucose (FPG) tests.
  • Oral glucose tolerance test (OGTT) with serial blood glucose measurements after ingestion of a standardized glucose load.
  • Glucose challenge (screening) test for gestational diabetes, with a single post‑load blood glucose measurement.
  • Random (non‑fasting) blood glucose testing.
  • Capillary blood glucose measurement using handheld blood glucose meters and disposable test strips.
  • Hemoglobin A1C (HbA1c) testing, which indirectly reflects average blood glucose over 2 to 3 months.
  • Glucose in urine tests, typically as part of urinalysis.

Test Preparation and Influencing Factors

  • Fasting requirements

    • Fasting blood glucose and OGTT generally require no food or drink (except water) for 8 to 12 hours before the test.
    • Random blood glucose, glucose challenge tests, A1C, and urine glucose tests usually do not require fasting.
  • Medications and substances

    • Certain medications can affect blood glucose levels; patients are typically advised to discuss all medicines, supplements, and insulin use with their clinician before testing.
    • Intravenous dextrose, recent high‑sugar meals, or alcohol intake can alter results, especially for non‑fasting tests.
  • Timing and recent food intake

    • Fasting tests are usually scheduled in the morning to standardize conditions and ensure adequate fasting.
    • Post‑prandial or OGTT measurements depend on precise timing after glucose ingestion (for example, 1 to 2 hours).
  • Physiological and health factors

    • Acute illness, stress, infections, or surgery can raise blood glucose levels.
    • Pregnancy alters glucose metabolism and requires specific protocols for gestational diabetes testing.
    • Kidney function and presence of glucosuria influence interpretation of urine glucose tests.
  • Technique and sample handling

    • For home monitoring, proper handwashing, use of a new lancet, and correct application of blood to the strip are important for reliable readings.
