Biomarker: LDL Cholesterol
Overview
Low-density lipoprotein (LDL) cholesterol is the fraction of cholesterol carried in the blood by LDL particles, which transport cholesterol to tissues for cell membrane maintenance and hormone synthesis. LDL carries most of the circulating cholesterol and, when elevated, promotes cholesterol deposition in arterial walls and the development of atherosclerotic plaque, so it is often called “bad” cholesterol. High LDL cholesterol is a major causal risk factor for atherosclerotic cardiovascular disease (ASCVD), including coronary heart disease and stroke, and lowering LDL reduces cardiovascular events. Clinically, LDL cholesterol is a central biomarker used to estimate cardiovascular risk, guide lipid-lowering therapy, and monitor treatment response. Very low LDL levels are uncommon but can occur with intensive lipid-lowering therapy or certain genetic conditions and are generally interpreted in context of overall health and treatment goals.
Clinical Use Cases
- Assessing baseline cardiovascular risk and guiding primary prevention strategies for ASCVD.
- Diagnosing and characterizing dyslipidemia and hypercholesterolemia, including suspected familial hypercholesterolemia when LDL is severely elevated.
- Monitoring response to lifestyle interventions and lipid-lowering medications such as statins, ezetimibe, and PCSK9 inhibitors.
- Determining LDL treatment targets in patients with established ASCVD, diabetes, chronic kidney disease, or multiple risk factors.
- Periodic screening for cholesterol abnormalities in adults and children based on age and risk-factor–based guidelines.
- Risk stratification using LDL levels as part of global risk scores and as a risk enhancer when markedly elevated (for example ≥160–190 mg/dL).

- Venous blood serum (most common).
- Venous blood plasma collected in appropriate anticoagulant tubes for some laboratory methods.
Measurement Methods
- Calculated LDL cholesterol (Friedewald equation) using total cholesterol, HDL cholesterol, and triglycerides in a fasting sample: LDL-C = total cholesterol − HDL-C − triglycerides/5; valid when triglycerides ≤400 mg/dL.
- Updated calculation formulas (for example Sampson/NIH equation) to improve accuracy at high triglycerides or low LDL levels.
- Direct homogeneous enzymatic assays that measure LDL cholesterol without calculation and are suitable when triglycerides are high or fasting is not possible.
- Ultracentrifugation-based reference methods used in specialized or reference laboratories for standardization and method validation.
Test Preparation and Influencing Factors
- Fasting requirements: Traditional lipid panels and LDL calculations usually require 9 to 12 hours of fasting (water allowed), especially when triglycerides are measured for calculated LDL.
- Nonfasting testing: Some guidelines and labs accept nonfasting samples, particularly when using direct homogeneous assays, though very high triglycerides can still affect results.
- Medications: Lipid-lowering drugs (statins, ezetimibe, PCSK9 inhibitors, fibrates), some diuretics, beta-blockers, steroids, and certain antiretroviral or immunosuppressive agents can change LDL levels and should be considered when interpreting results.
- Recent illness or major surgery: Acute illness, systemic inflammation, and major surgery can transiently lower LDL levels, potentially underestimating baseline cardiovascular risk.
- Diet and alcohol: Recent high-fat meals and excessive alcohol intake can raise triglycerides, which may interfere with calculated LDL and overall lipid interpretation.
- Body weight and metabolic factors: Overweight, obesity, metabolic syndrome, and diabetes are associated with higher LDL and other atherogenic lipoproteins.
- Age, sex, and genetics: LDL levels vary with age and sex, and genetic conditions such as familial hypercholesterolemia cause markedly elevated LDL independent of lifestyle.
