Biomarker: HDL Cholesterol

Overview

High‑density lipoprotein (HDL) cholesterol is the fraction of cholesterol carried in the blood by HDL particles, often called “good” cholesterol because it helps remove excess cholesterol from the bloodstream and transport it back to the liver for elimination. HDL plays a protective role against atherosclerosis and cardiovascular disease by participating in reverse cholesterol transport and is used, along with other lipids, blood pressure, diabetes status, and smoking history, to estimate overall cardiovascular risk. Higher HDL levels are generally associated with a lower risk of coronary artery disease, heart attack, and stroke, whereas low HDL is considered a risk factor and is one of the criteria for metabolic syndrome. Very low HDL levels may reflect factors such as poor diet, lack of physical activity, obesity, smoking, and certain medical conditions; very high HDL can occasionally be associated with underlying lipid disorders or other health issues. Clinically, HDL cholesterol is routinely measured as part of a fasting or non‑fasting lipid panel to guide cardiovascular risk assessment and prevention strategies.

Test Preparation and Influencing Factors

  • Often measured as part of a lipid panel that may require fasting for 9 to 12 hours, depending on the laboratory’s protocol and concurrent triglyceride testing requirements.
  • Recent food intake, especially high‑fat meals, can affect triglycerides and, indirectly, interpretation of HDL within the full lipid profile.
  • Medications, including estrogen therapy and certain lipid‑modifying drugs, can increase HDL levels.
  • Other medications, such as some beta‑blockers, anabolic steroids, and progestins, may lower HDL cholesterol (noted in clinical reviews and practice guidelines).
  • High alcohol intake can raise HDL but may reflect unhealthy drinking patterns and other metabolic effects.
  • Smoking, physical inactivity, obesity, and diets high in saturated fat or refined carbohydrates can lower HDL.
