Biomarker: Absolute Neutrophils
Overview
Absolute neutrophils, also known as the absolute neutrophil count (ANC), measures the actual number of neutrophils in a microliter of blood. Neutrophils are the most common type of white blood cell and act as the immune system’s first responders to bacteria and tissue injury, helping to control and resolve infections. A low ANC (neutropenia) reduces the body’s ability to fight off infections and increases the risk of serious bacterial illness, while a high ANC commonly reflects active infection, inflammation, or stress such as surgery, trauma, or corticosteroid use. Because neutrophils react quickly, the ANC is a clinically useful marker of acute illness, bone‑marrow function, and immune readiness.
Clinical Use Cases
- Evaluate suspected infection or sepsis, including febrile neutropenia in people receiving chemotherapy.
- Monitor immune status and infection risk in patients on immunosuppressive drugs, chemotherapy, or with certain bone‑marrow disorders.
- Help diagnose and follow conditions such as leukemia, myelodysplastic syndromes, autoimmune diseases, and chronic inflammatory disorders.
- Assess inflammatory states (e.g., acute infection, trauma, surgery, or severe stress) when the total white‑cell count is high.
- Support eligibility and safety criteria in oncology clinical trials that require a minimum ANC threshold.
Specimen Types
- Whole blood collected in an anticoagulant tube (typically EDTA, “purple top” tube).
- Venous blood (most common) or capillary blood (fingerstick or heelstick) in some point‑of‑care or pediatric settings.
Measurement Methods
- Complete blood count (CBC) with automated differential cell count, from which the ANC is calculated.
- Manual microscopic differential (blood smear) used to verify or supplement automated counts when results are abnormal or flagged.
- The ANC is usually derived from the total white‑blood‑cell count multiplied by the percentage of neutrophils (including segs and bands), or by adding absolute segmented neutrophils and absolute band neutrophils.
Test Preparation and Influencing Factors
- No routine fasting is required for an ANC check, unless the same blood draw is being used for other tests that do require fasting.
- Timing of collection can matter; acute infections, stress, and recent surgery or trauma can raise levels within hours, while chemotherapy or bone‑marrow suppression can lower levels over days.
- Certain medications can affect levels, including some chemotherapy drugs, antipsychotics (e.g., clozapine), antithyroid drugs, and other immunosuppressants.
- Physiological variables such as pregnancy (mild, non‑dangerous increase), smoking, and intense physical exertion can also transiently raise neutrophil counts.
- Infection type and severity, chronic inflammatory diseases, and underlying bone‑marrow problems can cause persistent high or low values.
